New Patient Information
We respect the value of your time and will make every effort to honor all time commitments with a minimum waiting time. Please extend the same courtesy to us by arriving promptly for your appointment. Please contact us at least 24 hours in advance if you are unable to keep a scheduled appointment. There will be a charge for repeated broken appointments.
If you are a new patient, please complete the following forms and bring them with you to your first appointment.
Payment Options
Ability to pay is of deep concern to many dental patients. We share your concern and are prepared to offer
you a number of payment options. We will discuss the payment plan best suited for you in privacy and assist
you with all the arrangements.
- If you have dental insurance, we file claims as a courtesy for our patients. However, any coverage denials will become the patient's responsibility. We will assist you to the best of our abilities in your appeal process.
- Payment is due at each appointment as well as fees not covered by insurance. Cash, checks and most major credit cards are accepted.
- We offer affordable financing plans through CareCredit.

We are conveniently located in the Missouri Professional Plaza in central Phoenix. Our office is in the two-story, gray building on the south side of the road, second floor. To gain access to the second floor, there is an elevator in the center of the building or stairs on either side of the building.
